An acceptable list of sources of vaccination records is below. Please ensure the documentation shows both your first and last name and a complete date of vaccination (month, day, year).
- State-based immunization systems (Students vaccinated in Michigan are able to request a copy of their MCIR record showing their vaccination history)
- County-based immunization systems
- Health System Portal (MyChart)
- Primary care physician office records (a blank form for your primary care physician to complete is available here)
- Pharmacy records
- Vaccination card or booklet
- Country or Ministry of Health records
